Job summary

SPECTRAFOR is seeking a dedicated Long-term Service Care Manager in the Town of Florida, New York. The role involves managing a caseload of members, completing assessments, and coordinating person-centered care, with a mix of home visits and remote work.

Responsibilities

• Conduct monthly and quarterly contacts with members.
• Perform in-depth member assessments and document care notes.
• Collaborate with caregivers, providers, and discharge planners.
• Monitor services and coordinate referrals.
