Job summary

Community Health Network is seeking a Remote Patient Monitoring LPN for Community ProCare at Home. This fully remote role supports patients enrolled in a remote monitoring program, reviewing transmitted data and initiating outreach according to clinical protocols.

You will collaborate with nursing leadership to improve chronic disease management, promote patient engagement, and ensure timely follow-up in a technology-enabled care model. Indiana LPN licensure and drivers license are required.

Qualifications

• Graduate of an accredited practical nursing program.
• Indiana LPN license required.
• Experience in ambulatory care or telehealth preferred.
• Driver's license with vehicle insurance required in Indiana.

Responsibilities

• Review and monitor patient-generated data from remote monitoring platform.
• Respond to care path questionnaires submitted by patients.
• Identify abnormal trends and follow clinical workflows for outreach.
• Coordinate outreach and obtain patient consent.
• Conduct outbound calls to assess symptoms and support adherence to care plans.
• Document patient interactions and interventions in the EHR.
• Coordinate follow-up care with providers and care teams.
• Educate patients on device usage and data transmission.
• Reinforce care path zones based on vital signs.
• Support quality, compliance, and safety initiatives for remote monitoring.
