Qualifications

Responsibilities

• Conduct preliminary health assessments for newly enrolled patients.
• Resolve patients' questions and create open dialogue.
• Identify and coordinate community resources for patient care.
• Provide education on chronic condition management.
• Assist with medication management and refills.
• Ensure timely delivery of services including Home Health and DME.
