Responsibilities

• Accept transfers to conduct preliminary health assessments.
• Resolve patients' questions and create an open dialogue.
• Identify and coordinate community resources for patients.
• Provide education on management of chronic conditions.
• Assist with medication management and adherence.
• Ensure timely delivery of critical services.
• Most of the shift requires being on the phone.
