Job summary

CareHarmony, Inc. is seeking an experienced Licensed Practical Nurse (LPN) for a 100% remote position in Alabama. The ideal candidate will have at least 3 years of patient-facing experience and be passionate about delivering high-quality care. Responsibilities include managing patient care, providing education on chronic conditions, and coordinating resources. Starting pay is $21/hr, with the potential to earn up to $28/hr based on production, plus benefits such as health insurance, PTO, and a 401k plan.

Qualifications

• At least 3 years of direct patient-facing work experience.
• Excellent written and verbal communication skills.
• Technical aptitude in using Microsoft Office Suite.

Responsibilities

• Manage patient census with a resolution-driven approach.
• Identify and coordinate community resources with patients.
• Provide patient education on chronic conditions management.
• Perform medication management and coordinate refills.
• Assist in ensuring timely delivery of services to patients.
• Resolve patients' questions and create an open dialogue.
• Assist/manage referrals and appointment scheduling.
