Qualifications

• At least 3 years of direct patient-facing work experience.
• Experience identifying resources and coordinating needs for chronic care management patients.
• Strong capability to manage patient census and resolve questions.

Responsibilities

• Manage patient census with a resolution-driven approach.
• Identify and coordinate community resources that benefit patient care.
• Provide patient education on chronic conditions management.
• Perform medication management, including reconciliation and refills.
• Assist in ensuring timely delivery of services.
• Manage referrals and appointment scheduling.
