Qualifications
At least 3 years of direct patient-facing work experience.
Experience identifying resources and coordinating needs for chronic care management patients.
Strong capability to manage patient census and resolve questions.
Responsibilities
Manage patient census with a resolution-driven approach.
Identify and coordinate community resources that benefit patient care.
Provide patient education on chronic conditions management.
Perform medication management, including reconciliation and refills.
Assist in ensuring timely delivery of services.
Manage referrals and appointment scheduling.
Skills
