Job summary

Solace is seeking licensed LPNs/LVNs to join our remote Patient Advocate program for Medicare members. This 1099 independent contractor role allows flexible scheduling—full-time, part-time, or somewhere in between—and is conducted entirely remotely via video or phone.

You will apply your clinical background to untangle patients' health needs, design care plans that address barriers like access to food and transportation, and help members navigate care across the system.

Qualifications

• Current LPN or LVN license in good standing.
• Experience caring for patients from diverse backgrounds.
• Ability to work remotely via video or phone.

Responsibilities

• Leverage LPN/LVN clinical background with Solace platform and standards.
• Turn empathy and attentive listening into patient relationships built on trust.
• Read patient situations, identify priorities, and shape care plans over time.
