Solace is seeking an experienced Registered Nurse (RN) Healthcare Advocate to guide patients through their healthcare journey. This 1099 contract role is 100% remote with flexible hours, including part-time or full-time options. You will help patients navigate complex health concerns, address SDOH, and resolve issues with empathy and action.

Based in the United States, you will build care plans, coordinate resources, and work through video or phone with patients to improve health outcomes.

Qualifications

• Registered Nurse (RN) license in good standing.
• Experience working with diverse patient populations.
• Empathy and strong patient advocacy.
• Clinical and organizational skills to balance multiple tasks.

Responsabilités

• Learn Solace's system, tools, and expectations.
• Build strong, trusting relationships with patients.
• Identify and prioritize patient needs and build care plans.
• Develop comprehensive patient care plans addressing social determinants of health (SDOH).
