Job summary

ConcertoCare is seeking a skilled Social Worker to join a remote, multidisciplinary care team. The role requires WA LICSW and residence within the PST time zone, with duties centered on behavioral health assessments, brief therapy, crisis intervention, and care coordination for patients in community settings.

Qualifications

• Master's degree in Social Work is required.
• Current Washington independent licensure required.
• 2+ years of healthcare/behavioral health experience.

Responsibilities

• Provide behavioral health assessments and brief evidence-based therapy via telehealth.
• Deliver crisis intervention and treatment planning.
• Coordinate care with multidisciplinary teams and primary providers.
• Document encounters and treatment plans in electronic health records.
• Support medication management in collaboration with prescribers.
• Ensure culturally and linguistically appropriate care.
