Job summary

ConcertoCare is seeking a Social Worker (LICSW) to join a fully remote clinical team serving patients in community settings. The role requires WA licensure and residence within the PST time zone.

Responsibilities include behavioral health assessments, brief evidence-based therapy, crisis intervention, treatment planning, and care coordination for medically and socially complex patients. The position offers base salary of $85,000 to $100,000 plus annual bonus, with comprehensive benefits

Qualifications

• Master's degree in Social Work.
• Current Washington independent licensure required.
• 2+ years of healthcare/behavioral health experience.
• Strong problem-solving and organizational skills.
• Geriatric care experience highly desired.
• Ability to communicate effectively in writing and verbally.
• Proficiency with Microsoft Office and EMR systems.

Responsibilities

• Provide behavioral health assessments and brief therapy via telehealth to an assigned caseload.
• Support psychotropic medication management and coordinate with prescribing providers.
• Participate in caseload reviews and clinical team meetings; communicate findings clearly.
• Document patient encounters and referral activities in the electronic health record.
• Ensure care respects patients' diverse values, beliefs, and backgrounds.
• Share knowledge and resources as a subject matter expert within the behavioral health team.
• Maintain professionalism and perform other duties as assigned.
