Job summary

ConcertoCare Social Worker role is fully remote but must reside in the PST time zone and hold an active WA LICSW. The team delivers behavioral health assessments, brief therapy, crisis intervention, and care coordination for medically and socially complex patients.

The position offers base salary $85,000–$100,000 plus annual bonus, plus medical/dental/vision coverage and a 401(k). 15 days of PTO and 11 paid holidays are included.

Qualifications

• Master’s degree in Social Work.
• Current Washington independent licensure required.
• 2+ years of healthcare/behavioral health experience.
• Strong problem-solving and organizational skills.
• Geriatric care experience highly desired.
• Ability to manage patient complexity and multiple clients with diverse needs.
• Excellent written and verbal communication; team oriented.
• Proficient in computer skills including EMR and Microsoft Office.

Responsibilities

• Provide behavioral health assessments and brief evidence-based therapy via virtual visits.
• Deliver crisis intervention, treatment planning, and care coordination.
• Coordinate with prescribing providers and clinical teams on psychotropic issues.
• Document patient encounters and referrals in electronic health records.
• Attend caseload reviews and clinical team meetings; communicate clearly.
• Respect and incorporate diverse values, beliefs, and backgrounds in care.
