Job summary

CareHarmony, Inc. is seeking an experienced Licensed Practical Nurse (LPN) to join our remote care coordination team.

This full-time, 100% remote role requires at least 3 years of direct patient-facing experience and an active Multi-State/Compact LPN license. You will manage patient census, identify resources, educate patients on chronic condition management, perform medication management, and coordinate referrals to Home Health, DME, and infusion services.

Qualifications

• 3+ years of direct patient-facing nursing experience.
• Proficiency with Microsoft Office and HIPAA compliance.

Responsibilities

• Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical care.
• Identify and coordinate community resources for patients who would benefit their care.
• Provide patient education and health literacy for chronic condition management.
• Perform medication management, including identifying concerns, reconciliation, adherence, and refills.
• Assist in ensuring timely delivery of Home Health, DME, Home Infusion, and other services.
• Resolve patient questions and create open dialogue to understand needs.
• Assist/manage referrals and appointment scheduling.

CareHarmony is seeking an experienced Licensed Practical Nurse to work 100% Remote – LPN Nurse (LPN) (LVN) with at least 3 years of direct patient-facing work experience; that thrives in a fast-paced environment, is self-motivated, has impeccable attention to detail, and values the impact they can have on a patient’s healthcare journey.

Requirements

Physical Requirements:

• The position starts at $21/hr with the ability to earn up to $28/hr based on production
