Ascend Healthcare is seeking a Licensed Independent Therapist (LCSW, LPCC, LMFT) to provide counseling remotely via telemedicine to patients through Community Health Center partners. A “say yes” first provider, we treat patients where they are and guide them toward recovery and stabilization.

The position requires an active California license and remote-work readiness. Opportunity to work with trauma modalities and a collaborative, remote-first team with perks and a monthly bonus program.

Competenze

• Active, unrestricted California license required.
• Experience delivering therapy remotely via telemedicine.
• Training in evidence-based trauma modalities preferred.
• Strong documentation and crisis intervention skills.

Mansioni

• Provide psychotherapy via telemedicine to patients of Community Health Center partners.
• Review and sign psychosocial assessments, diagnoses, and treatment plans; maintain clinical records.
• Document all services and patient activities per best practices and regulatory requirements.
• Conduct crisis intervention for individuals and families as needed.
• Participate in regular case planning with the Multidisciplinary Team.
• Engage in supervision and ongoing education to maintain competency.
• Provide case management services to promote recovery and community reintegration.
• Perform additional duties as assigned.
