Job summary

CVS Health is hiring an Associate Manager of Community Health Worker (CHW) Pilot to lead implementation, oversight, and ongoing management focused on reducing disparities among Medicaid members with chronic conditions. The role oversees CHWs, coordinates with care teams, and drives improvements in engagement and outcomes.

The position requires 3–5 years in care management or population health, supervisory experience, and knowledge of SDoH.

Qualifications

• 3–5 years of care management or population health experience.
• Supervisory or team leadership background.
• Experience managing programs for high‑risk or underserved populations.
• Strong knowledge of SDoH, care coordination, and community interventions.
• Experience tracking performance metrics and improving utilization.
• Excellent communication and collaboration skills.

Responsibilities

• Oversee CHW pilot implementation and daily operations.
• Supervise CHWs, assign caseloads, and ensure performance.
• Ensure timely member outreach and required assessments.
• Drive diabetes management and primary care utilization.
• Connect members to SDoH resources and manage referrals.
• Coordinate with interdisciplinary care teams to plan member care.
• Lead case reviews, rounds, and performance discussions.
• Monitor ED use, inpatient admissions, and quality metrics.
• Identify risks and gaps; implement corrective actions.
• Ensure documentation quality in care management systems.
• Support pilot evaluation with data reporting and analysis.
• Promote CHW workforce development and retention.
