Job summary

CVS Health is seeking an Associate Manager of Community Health Worker to lead the CHW pilot focused on reducing disparities among Medicaid members with diabetes or chronic conditions. This role oversees CHW staff, ensures integration with interdisciplinary care teams, and aligns with program goals.

The position requires RN or licensed social worker/therapist, 3–5 years of related experience, and a 40-hour week; work is remote anywhere in the USA with EST hours.

Qualifications

• 3–5 years in care management or Medicaid program operations.
• Experience supervising teams.
• Experience managing programs for high-risk/underserved populations.
• Strong understanding of SDoH and care coordination.
• Experience driving quality and utilization metrics.
• Excellent communication and analytical skills.

Responsibilities

• Oversee CHW pilot implementation and day-to-day operations.
• Supervise CHWs and assign caseloads up to ~30 members.
• Ensure timely member outreach and required assessments.
• Drive CHW activities to close care gaps and increase primary care use.
• Coordinate with interdisciplinary care teams for member plans.
• Monitor pilot metrics: ED visits, admissions, primary care utilization.
• Oversee documentation quality in care management systems.
• Support program evaluation with data reporting.
• Promote CHW workforce development through coaching.
• Ensure compliance with Medicaid and DMAHS requirements.
