Job summary

CVS Health is hiring a Community Health Worker to support a CHW Pilot addressing health disparities and improving outcomes among Medicaid members with Type 2 diabetes. The position involves field work in Essex County, NJ, with a typical schedule of 40 hours per week and a focus on engaging underserved communities in culturally competent care.

The CHW will liaise with members, care teams, and community resources to improve access to care, support self-management, and reduce unnecessary

Qualifications

• High school diploma or equivalent required.
• Associate's or Bachelor's in Public Health or Social Work preferred.
• Experience in community health or care management.
• Experience engaging underserved populations.
• Ability to document activities accurately in systems.

Responsibilities

• Conduct telephonic, virtual, and face-to-face outreach with assigned members.
• Maintain an active caseload (up to ~30 members) with engagement duration based on needs.
• Complete screenings and assessments, including engagement and health equity screeners.
• Develop and implement action plans with members and care teams.
• Provide education on Type 2 diabetes management and healthy behaviors.
• Identify gaps in care and support preventive and follow-up services.
• Connect members to community resources addressing SDoH needs.
• Assist with navigating primary and specialty care appointments.
• Participate in team meetings and care coordination activities.
• Document interactions and interventions in required systems.
• Support member engagement in pilot initiatives and community events.
• Promote culturally competent care aligned with community needs.
