Job summary

Peregrine Health Corporate is seeking a Licensed Clinician (LCSW or LPC) for a remote, full-time clinical role. You will conduct biopsychosocial assessments, provide individual, family, and group therapy, and develop personalized treatment plans in collaboration with clients and the care team.

Responsibilities include crisis intervention, care coordination with physicians and community resources, thorough documentation in the EHR, and adherence to HIPAA and state regulations.

Qualifications

• Active LCSW license or LPC/LMHC certification is required.
• 3+ years of post-licensure clinical experience in behavioral health.
• Comfort providing therapy independently within a supported care model.
• Strong clinical judgment in assessment, diagnosis and treatment planning.
• Clear, professional communication and collaboration with care teams.
• Comfort using telehealth platforms and clinical documentation tools.
• Commitment to full-time work and continuity of care.

Responsibilities

• Direct clinical services including biopsychosocial assessments and therapy.
• Develop and implement individualized treatment plans with clients and care team.
• Monitor progress and adjust treatment plans as needed.
• Crisis intervention, safety planning, and stabilization services.
• Care coordination with physicians, nurses, case managers, and community health workers.
• Documentation in EHR, HIPAA compliance, and quality improvement participation.
