Job summary

Transformcap is seeking an LCSW Clinical Specialist II for Synchronous AHPC Partnership (CA) - Remote. The role advances a world-class clinical experience for Participants, collaborating with the Health Care Industry and clinical teams to improve quality of life and wellbeing.

You will support participants and teams, facilitating sessions, note-taking, and care planning while staying current on clinical care advances and tele-health practices. This is a fully remote, impactful clinical role.

Qualifications

• Licensed LCSW with ability to practice without supervision.
• Experience with tele-health platforms is preferred.
• Comfortable with technology and virtual sessions.
• Ability to work remotely from a private location.

Responsibilities

• Provide 30–37 hours of direct counseling per week.
• Develop treatment plans and documentation.
• Attend weekly team meetings and care coordination with Customer Success.
• Escalate participant concerns via operations as needed.
• Support outreach, materials, and industry presentations.
