Job summary

Healthmap Solutions is seeking a Care Navigator to perform case management for kidney health programs, coordinating care across physicians, nurses, and other providers to improve outcomes. The role emphasizes member education, data entry, and adherence to policies in a multidisciplinary team.

The ideal candidate is a licensed RN with case management experience and knowledge of nephrology or related payers (Medicare/Medicaid).

Qualifications

• Active, unrestricted RN license required.
• Bachelor’s degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM preferred
• Three (3) years of experience in case management preferred
• Experience working with nephrology or cardiology patients preferred
• Experience with Medicare and Medicaid preferred

Responsibilities

• Handle inbound and outbound calls delivering world-class service to our members
• Educate kidney health and related co-morbid conditions as well as dialysis and transplant options
• Engage members into HealthMap’s Kidney Health Program
• Follow up with members based on complexity and cadence by policy
• Serve as patient advocate for responding and resolving concerns or barriers
• Utilize community resources and programs in care planning
• Coordinate care between patient, support network, treating physician, and other providers
• Notify providers of identified patient needs per policy
• Comply with HIPAA privacy laws and all regulations
• Navigate technical applications - Excel, OneNote, Outlook, Word
• Support after hours and across time zones as needed
• Drive patient and families in their own care and to support self-management
