Job summary

Advanced Behavioral Health, Inc. in Connecticut is seeking an Intensive Care Manager (ICM) to support clients with severe mental illness transitioning from nursing home care back to the community.

This hybrid role combines remote work with frequent in-person visits across southwestern CT (Bridgeport–Stamford area) and requires reliable transportation. The ICM will coordinate recovery plans, conduct assessments, arrange discharge planning, and collaborate with providers while maintaining HIPAA

Qualifications

• Master's Degree in social work, psychology, or related field.
• Licensure: LCSW, LPC, or LMFT, or licensed eligible.
• Minimum 3 years of behavioral health clinical experience.
• Experience delivering community-based services.
• Knowledge of CT/state/federal health and vaccine requirements.
• Understanding of best practices in behavioral health for special needs populations.

Responsibilities

• Travel statewide in CT to assess and evaluate potential clients in nursing homes.
• Complete MFP/DSS paperwork as required.
• Coordinate discharge plans and recovery plans with community services.
• Review recovery plans and service authorizations.
• Maintain confidentiality and HIPAA compliance.
• Attend case conferences and interagency planning meetings.
