Job summary

Advanced Behavioral Health (ABH®) is seeking an Intensive Care Manager (ICM) to coordinate recovery support for clients with severe mental illness transitioning from nursing home care to community living. The hybrid role involves home-based work with travel across CT to visit clients in nursing homes, coordinate discharge plans, and lead clinical initiatives within the WISE Program.

Qualifications

• Master's Degree in related field (social work, psychology, etc.)
• Licensure: LCSW, LPC or LMFT, or licensed eligible (ABH® will assist with clinical hours)
• 3 years minimum in behavioral health clinical experience
• Experience in delivery of community-based services
• Compliance with CT and federal health and vaccine requirements
• General knowledge of best practices in behavioral health for special needs populations

Responsibilities

• Assumes responsibility for a designated client caseload
• Travels statewide to assess and evaluate potential clients in nursing homes transitioning to the community
• Completes MFP/DSS paperwork as required
• Provides clinical expertise and collaborates with the team
• Coordinates recovery plan reviews and care coordination for assigned clients
• Maintains HIPAA compliance and confidentiality of client information
• Attends case conferences and professional development activities
• Lends clinical expertise to WISE program staff
