Job summary

Neighborhood Health Plan of Rhode Island is seeking an Integrated Care Manager (ICM) to work with the integrated care team. You will manage care for high-risk member populations, conducting assessments, developing care plans, coordinating services, and monitoring outcomes to ensure quality and cost-effective member-centered care.

The role requires current RI licensure in a clinical field (RN or behavioral health) or eligibility with licensing in process, plus at least two years in care

Qualifications

• Active, current RI license as RN or behavioral health professional (LMHC/LCDP/LCSW/LICSW) or eligible with license in process.
• Minimum 2 years in care management, behavioral health, or community health.
• Proficiency with PC-based software programs for documentation and case tracking.
• Strong organizational and interpersonal skills; customer service orientation.
• Ability to work independently and prioritize multiple activities.
• Must have reliable transportation, driver’s license, and auto insurance.

Responsibilities

• Practice care management within licensure scope.
• Coordinate care with the integrated care team for high-risk member populations.
• Conduct comprehensive assessments and develop care plans with goals with members and care partners.
• Communicate with providers to facilitate needed care and coordinate services.
• Monitor outcomes, adjust plans, and identify gaps in care.
• Document all activities in the care management system.
• Provide supervision to outreach staff as appropriate and participate in outreach activities.
