Job summary

BlueCross BlueShield of South Carolina is hiring an Intensive Care Management Coordinator. This full-time, remote position requires travel within South Carolina to conduct member visits, with 8:30am-5:00pm EST hours and weekly on-site requirements as needed.

The role focuses on assessment, planning, coordination, and advocacy for members with behavioral health needs. Ideal candidates have a 4-year degree in nursing or related field and behavioral health experience, plus proficiency in Microsoft

Qualifications

• Bachelor's degree or higher with 45 hours of specialty training in Behavioral Health.
• Experience with case management and/or utilization review in behavioral health preferred.

Responsibilities

• Perform assessment, planning, coordination, monitoring, evaluation, and advocacy for assigned members.
• Collaborate with families and other healthcare professionals to coordinate care.
• Travel within South Carolina to conduct member visits.
• Conduct in-person assessments in member homes or least restrictive environments.
