Job summary
BlueCross BlueShield of South Carolina is seeking an RN Behavioral Health Concierge Care Manager to support over 2 million members nationwide. This full-time remote role focuses on high-touch care coordination for high-risk populations, education on prior authorizations, and developing individualized care plans to improve outcomes.
Work hours are 8:00am-5:00pm EST, Monday–Friday; remote home-office setup required. Must hold RN licensure in the U.S.
Qualifications
- Active, unrestricted RN licensure in the United States or compact multi-state license.
- Case management experience in health care; CMS/quality processes knowledge.
- Proficiency with Microsoft Office and word processing software.
- Ability to work independently and manage complex cases with discretion.
- Strong communication, organization, and presentation skills.
Responsibilities
- Directs day-to-day high-touch member management and care coordination.
- Coordinates clinical services with providers, facilities, and social workers.
- Participates in team training and quality assurance activities.
- Prepares for external audits and supports program goals.
