Job summary

Reliant Medical Group is seeking a Per Diem Nurse Practitioner for the Behavioral Health Integrated Substance Use team in Worcester, MA. The role blends telemedicine with on-site care to support patients in recovery using evidence-based medications and new digital therapeutics.

The ideal candidate has MA APRN licensure, NP board certification, and SUD experience. The position offers a hybrid schedule and opportunities for professional growth within OptumCare.

Qualifications

• Massachusetts APRN license required.
• Board certification as Nurse Practitioner.
• DEA license, X-Waiver and MA MCSR certificate (or in process).
• 1–3 years postgraduate experience treating patients with substance use disorders and dual diagnosis.

Responsibilities

• Performs comprehensive physical assessments for SUD patients and related conditions.
• Takes detailed history including SUD and comorbidities; conducts appropriate exams.
• Orders labs as needed and assesses severity to determine care needs.
• Implements evidence-based plans including MOUD, with referrals as appropriate.
• Communicates with BH and primary care teams; maintains precise documentation.
• Contributes to program development, training, and BH/primary care initiatives.
• Provides supervision to Substance Use team staff as appropriate.
• Supports telemedicine and on-site service delivery with reliable attendance.
• Participates in on-call coverage and after-hours care as needed.
