St. Tammany Health System seeks an Outpatient Care Coordinator RN (Per Diem) to coordinate care across the continuum, assess patient needs, and collaborate with care teams in Covington and Madisonville, LA.

The role emphasizes improving outcomes and reducing resource over-utilization. RN licensure in Louisiana is required, BSN preferred, with two years of acute care, ED, case management, or utilization management experience preferred.

Qualifications

• Graduate from an accredited nursing program; BSN preferred.
• Current Louisiana RN licensure.
• Knowledge of post-acute providers and community resources.
• Strong planning, organizing, leadership and communication skills.
• BLS certification through an ILCOR-recognized provider (e.g., AHA, Red Cross).
• Two years of Acute Care Hospital, ED, Case Management, and/or Utilization Management experience preferred.

Responsibilities

• Assess, plan, implement, coordinate and evaluate options and services to meet health needs.
• Facilitate progression of care and monitor patient progress to ensure effective plan of care.
• Help patients overcome barriers to access the continuum of care and social determinants of health.
• Coordinate care for patients with acute/chronic conditions and multiple comorbidities.
