Job summary

Harbor Healthcare System in Lake Charles, LA is seeking a full-time Registered Nurse Case Manager for an outpatient role. The RN will be integral to the interdisciplinary care team, handling admissions, assessments, discharges, and care coordination for each patient and family.

The position requires an active Louisiana RN license, strong communication skills, and the ability to travel as needed. Prior hospice/home health experience is preferred. A clean background and driving record are required.

Qualifications

• Licensed RN in Louisiana – required.
• 1+ years' experience as an RN – preferred.
• Experience with Hospice or Home Health – preferred.
• Effective written and verbal communication.
• Ability to travel, must have reliable transportation.
• Clean background, drug screen, and driving record.

Responsibilities

• Coordinate admissions, assessments, and discharges.
• Coordinate care for each patient and family as part of the care team.
• Manage pronouncements and ensure appropriate care planning.
