Qualifications

• Proficiency with MS Office applications.

Responsibilities

• Conduct post-discharge outreach to reduce readmissions.
• Assess, plan and implement individualized care strategies.
• Coordinate with physicians, case managers and community resources.
• Develop and monitor care plans aligned with payer requirements.
• Document assessments, care plans and referrals for continuity of care.
• Support members with barriers to in-home care and transportation.
