Essen Health care is seeking a Remote House Calls Nurse Practitioner to provide virtual clinical oversight and patient assessments for home-based care across NJ, OH, PA, NE, and TX. This fully remote role emphasizes quality, efficiency, and patient-centered care.

The position requires an active NP license in the listed states, ANCC or AANP certification, and a minimum of 2+ years of clinical NP experience, preferably with house calls or geriatrics. Telehealth experience is highly valued.

Qualifications

• Active NP license in the required state(s).
• National NP certification (ANCC or AANP).
• Minimum 2+ years of clinical NP experience.
• Experience in house calls, home health, geriatrics, primary care, or internal medicine preferred.
• Telehealth or remote care experience preferred.

Responsibilities

• Conduct telehealth visits for patients enrolled in the House Calls program.
• Manage chronic conditions and coordinate care remotely.
• Perform care management assessments, follow-ups, and care plan adjustments.
• Review patient histories, labs, imaging, and diagnostic results.
• Support medication management and reconciliation.
• Coordinate with primary care providers and specialists.
• Document visits and clinical decisions accurately in the EMR.
• Ensure compliance with clinical protocols and regulatory requirements.
• Participate in interdisciplinary case reviews and quality improvement.
