Qualifications

• Master's Degree in a Nurse Practitioner Program, focusing on Family or Adult Geriatrics.
• Current New York State License as a Nurse Practitioner.
• National Board Certification required.
• Bilingual (Spanish Speaking) is a plus.
• EMR/EHR experience is a plus.

Responsibilities

• Conduct comprehensive assessments of patients' health status during home visits.
• Diagnose medical conditions and develop treatment plans.
• Collaborate with other healthcare providers for continuity of care.
• Educate patients and families on health management and medication adherence.
• Maintain accurate documentation in electronic health records.
• Schedule follow-up visits to monitor patient progress.
• Assess and manage urgent medical issues during home visits.
