Job summary

Haven Hospice is seeking a compassionate Triage RN to join our team for after-hours support from home. The role involves answering hospice calls during nights, weekends, and emergencies, scheduling nurse visits, and coordinating on-call staff to ensure timely patient care.

The ideal candidate is an RN with hospice experience, holds a state license, and has strong clinical assessment and communication skills. A driver's license and reliable vehicle are required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• 1 year hospice nursing experience required, 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Prior experience with triage, preferred.

Responsibilities

• Answer hospice calls during nights, weekends, holidays, and emergencies.
• Help schedule nurse visits, handle emergency patient admissions, and dispatch on-call staff.
• Talk with patients and caregivers to assess needs and provide support over the phone.
• Communicate with doctors about patient updates and get new care orders if needed.
• Give instructions and guidance based on each patient’s care plan.
• Coordinate after-hours needs such as equipment, supplies, and medications.
• Work with facility staff to support their patients during off-hours.
• Document all calls, care updates, and decisions clearly and accurately.
• Ensure all care and procedures follow state and federal rules.
