Haven Hospice in Gainesville, FL is seeking a compassionate Hospice Triage Registered Nurse for after-hours and weekend coverage. The role focuses on coordinating care, triage calls, scheduling nurse visits, and ensuring safe patient support from home when possible.

Qualifications

• Graduate of an accredited nursing school with current licensure.
• 1 year hospice nursing experience required, 3+ years preferred.
• Nursing Compact RN license required.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Answer hospice calls during nights, weekends, holidays, and emergencies.
• Help schedule nurse visits, handle emergency patient admissions, and dispatch on-call staff.
• Talk with patients and caregivers to assess needs and provide support over the phone.
• Communicate with doctors about patient updates and get new care orders if needed.
• Give instructions and guidance based on each patient’s care plan.
• Coordinate after-hours needs such as equipment, supplies, and medications.
• Work with facility staff to support their patients during off-hours.
• Document all calls, care updates, and decisions clearly and accurately.
• Ensure all care and procedures follow state and federal rules.
