Job summary

Sequoia Hospice, part of The Pennant Group, seeks a Remote Triage RN to perform initial and ongoing assessments by phone and guide hospice care needs. The role emphasizes independent work, coordination with families, and delivering skilled nursing support in a remote setting.

The nurse will manage a caseload, develop care plans, and provide education while using Home Care Home Base. This position requires California licensure, hospice experience, and CPR certification.

Qualifications

• Must have a valid RN license in the state of California.
• Minimum of two years of hospice experience.
• Demonstrates nursing skills per competency checklist.
• Maintains CPR certification.

Responsibilities

• Completes initial, comprehensive and ongoing assessments via phone to determine hospice needs.
• Develops a care plan based on nursing diagnosis and involves patient and family.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting nursing and related needs.
• Acts as Case Manager coordinating patient care for assigned caseload.
• Provides health care instructions and supports activities of daily living.
