Job summary

Sequoia Hospice is seeking a Remote Triage Registered Nurse (RN) to perform initial and comprehensive assessments by phone, determine hospice needs, and coordinate care for a remote, night shift setting (5pm–8pm PST, Mon–Thu). California RN license and hospice experience are required.

Qualifications

• Must have a valid RN license in the state of California.
• Minimum of two years of hospice experience.
• Experience with Home Care Home Base preferred.
• Strong observation, problem-solving, and communication skills.

Responsibilities

• Perform initial and comprehensive assessments by phone to determine hospice needs.
• Provide physical assessment history and nursing care using the nursing process.
• Develop care plans with goals based on diagnoses and palliative actions.
• Coordinate patient care as Case Manager for assigned caseload.
• Counsel patients and families on nursing needs and home care.
• Administer medications as prescribed and educate on care plans.
