Job summary

Amedisys is seeking a compassionate Registered Nurse to support hospice patients remotely. The role emphasizes telephonic triage, plan-of-care updates, and collaboration with pharmacies and DME vendors to ensure optimal patient outcomes.

The ideal candidate has at least 1 year of hospice experience, an active RN license, CPR certification, and comfort with technology. This is a part-time, remote position with weekend and holiday considerations.

Qualifications

• Current, unencumbered RN license for the state of assignment.
• Minimum 1 year hospice or home health nursing experience.
• CPR certification and tech-savvy for telehealth coordination.

Responsibilities

• Assess hospice needs telephonically using patient plans of care.
• Utilize EHR to develop and update patient care plans.
• Provide education and hospice support telephonically per plan of care.
• Collaborate with pharmacies, DME vendors and other agencies for patient management.
• Facilitate delivery/maintenance of provided medical equipment.
• Assign visits, admissions and follow-up calls to on-call staff.
• Submit accurate documentation in real-time to support continuity of care.
• Participate in agency performance improvement initiatives.
