Job summary

Moments Hospice is seeking an experienced RN for a fully remote Triage RN role. You will provide after-hours triage, clinical assessments, and care coordination via telephone, supporting patients and families with end-of-life needs.

Collaboration with physicians, facilities, and internal teams ensures timely interventions and high-quality, compassionate care. Ideal candidates have hospice experience and are proficient with HCHB.

Qualifications

• Active RN license with no restrictions
• Hospice experience required
• Home Care Home Base (HCHB) experience preferred

Responsibilities

• Provide after-hours triage and clinical assessment via phone
• Support patients and families with symptom management and end-of-life needs
• Coordinate care with physicians, facilities and internal teams
• Dispatch visits and coordinate services with vendors
• Deliver clear, compassionate education and support to families
• Accurately document interactions, care plans and outcomes in the EMR
• Review team documentation for accuracy and compliance
• Partner with Telecare staff to ensure positive patient outcomes

Job description

TheMoments Hospice Triage RNis a key member of the interdisciplinary team, providing after-hours clinical support to patients and families. In this fully remote role, you will assess needs, provide guidance, and coordinate care using the nursing process and Telecare protocols.

This position is ideal for an experienced hospice RN who is confident working independently while delivering compassionate, patient-centered care.

What You’ll Do

• Provide after-hours triage, clinical assessment, and interventions via phone
• Support patients and families with symptom management and end-of-life care needs
• Coordinate care by collaborating with physicians, facilities, and internal teams
• Dispatch visits and coordinate services with vendors as needed
• Deliver clear, compassionate education and support to families
• Accurately document all interactions, care plans, and outcomes in the EMR
• Review team documentation for accuracy and compliance
• Partner with Telecare staff to ensure positive patient outcomes

Required Qualifications:

• Active RN license (no current or pending restrictions)
