Job summary

Roze Room Hospice of the Valley in Reseda is seeking an experienced triage registered nurse to deliver skilled and palliative care assessments and coordinate services with the hospice team after hours, weekends and holidays.

The nurse will process incoming calls from families and patients, update the plan of care, manage pain and symptoms, and provide education and support to patients and caregivers.

Qualifications

• RN licensed in California.
• Hospice/Palliative nursing certification preferred.
• Minimum 1 year of nursing experience within the last 3 years.
• Hospice experience preferred.

Responsibilities

• Provide skilled and palliative care assessments.
• Coordinate services with the hospice team after hours, weekends and holidays.
• Process incoming calls from families and patients.
• Update the plan of care and follow prescribed treatments.
• Educate and support patients and caregivers.
