Job summary

Southeast Massachusetts Council on Advocacy (SEMCOA) seeks a Home Visitor to support Healthy Families Brockton families. The role involves in-home visits, patient coaching, and family service planning under SEMCOA and the Healthy Families Massachusetts program.

The position is full-time with some evenings and weekends, offering remote/hybrid eligibility and a competitive pay range based on experience and education. Massachusetts driving license and reliable transportation are required.

Qualifications

• Must present as courteous and professional at all times.
• Demonstrated flexibility and ability to perform multiple tasks.
• Excellent communication skills, including the ability to organize ideas clearly.
• Ability to organize and prioritize a demanding workload.
• Knowledge of basic computer skills and software, including learning electronic record systems.

Responsibilities

• Provide consistent, strength-based home visits based on participant needs.
• Complete required assessments to ensure appropriate service delivery.
• Assist in development of Family Service Plan and monitor progress.
• Plan curriculum according to participant needs and interests.
• Maintain records per HFM policies and participate in supervision.
