Compassus is seeking a Remote Home Health Registered Nurse - Triage to triage patient and facility calls after hours and facilitate referrals as needed. This role supports field staff, coordinates admissions, and collaborates with physicians to ensure timely patient care.

The position requires an Associate or Bachelor’s degree in Nursing, an active Compact RN license, and CPR certification, with a preference for 1 year of RN experience in clinical or home health settings.

Responsabilidades

• Accepts and facilitates new referrals for home health and hospice patients.
• Acts as liaison to provide community education for referral sources, patients and families.
• Provides accurate and responsive service to calls received during weekend hours.
• Collaborates with physicians and obtains orders as needed to facilitate patient care.
• Provides guidance and support to field staff during hours worked.
• Assists in scheduling patient admissions, routine visits, and PRN visits for nurses, therapists, and home health aides.
• Monitors telehealth as needed.
• Performs other duties as assigned.

Ability to write reports, business correspondence, and procedure manuals. Able and willing to obtain additional licensures.Current CPR certification required.State Specific RequirementsLouisianaTwo (2) years of full time experience as a Registered Nurse; or two (2) years of clinical experience in home health required.Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Routine use of standard medical equipment and tools associated with clinical care is essential.
