Job summary

Unlimited Health Care Services Inc. is seeking a compassionate Physical Therapist to join our home health team in Fort Lauderdale and East Broward County. This per diem 1099 role offers flexible scheduling and the chance to provide one-on-one care in patients’ homes, with meaningful impact on mobility, independence, and quality of life.

Qualifications

• Active Florida Physical Therapist license in good standing.
• Doctor of Physical Therapy (DPT) from an accredited program.
• Current CPR/BLS certification.
• Strong clinical assessment and problem-solving skills.
• Excellent communication and interpersonal skills.
• Valid driver's license and reliable transportation for patient home visits.
• Current auto insurance.
• Ability to work independently and manage visits.
• Compassionate, patient-centered approach to care.

Responsibilities

• Perform comprehensive physical therapy evaluations and assessments
• Develop individualized treatment plans based on each patient's needs and goals
• Provide skilled physical therapy interventions in the home setting
• Implement therapeutic exercises and activities to improve strength, mobility, balance, coordination, and functional independence
• Evaluate patient progress and modify treatment plans as appropriate
• Educate patients and caregivers on therapeutic exercises, mobility, positioning, safety and home exercise programs
• Instruct patients and caregivers in techniques to promote safe mobility and independence
• Communicate patient progress and changes in condition with physicians, nurses and other members of the care team
• Complete accurate and timely clinical documentation for each visit
• Participate as an active member of the interdisciplinary care team
