SCAN Group is seeking a qualified HEDIS Nurse to review medical records and support NCQA reporting initiatives. The role involves outreach to members and providers, data abstraction, and coordinating with the project team to improve medication adherence and care gaps.

Qualifications

• RN or LVN license required.
• 2-3 years of related experience.
• 0-1 years of leadership experience.
• Knowledge of relevant policies, procedures, and practices.

Responsibilities

• Abstracts clinical data from medical records per NCQA guidelines and internal procedures.
• Communicates with members and providers regarding Quality Initiative projects and enters findings into the designated database.
• Documents all interactions with members per project manager specifications.
• Coordinates with HEDIS staff to update and track medical records for reporting purposes.
