Job summary

SCAN Health Plan seeks a HEDIS Nurse to review medical records and support NCQA-driven quality initiatives. This remote role coordinates with members and providers to collect complete data, focusing on medication adherence and preventive screenings.

The RN or LVN will abstract clinical data, document interactions, and participate in interdepartmental efforts to meet project timelines, with opportunities to expand responsibilities in a mission-driven, remote environment.

Qualifications

• Associate's degree or equivalent experience or technical degree.
• Registered Nurse (RN) or Licensed Vocational Nurse (LVN) required.
• 2–3 years of related experience.
• 0–1 year of leadership experience.

Responsibilities

• HEDIS duties: abstract data from medical records per NCQA guidelines and internal procedures.
• Improve quality of care by communicating with members and providers on Quality Initiative projects.
• Enter findings into designated database and document interactions with members.
• Coordinate with HEDIS staff to update and track records for reporting.
• Outreach duties: support projects across departments and review records to fulfill requirements.
