Qualifications

• High school diploma or GED required; associate degree preferred.
• 2+ years in healthcare-related fields required; clinical health care or managed care experience preferred.
• Experience with Electronic Health Records required; CPT II codes experience a plus.
• Proficiency in Microsoft Word, Excel, PowerPoint and Outlook.

Responsibilities

• Understand CMS, HEDIS, NCQA, CAHPS, HOS, and HIPAA principles.
• Support Quality Management with HEDIS, CMS Star Rating, and quality reporting.
• Provide telephonic outreach to patients, providers, and pharmacies to discuss optimizations.
• Improve patient experience and transitions to achieve better healthcare outcomes.
• Participate in data collection from medical records and claims surveillance.
• Lead Quality Fairs, Diabetic Days, and P3 events to close gaps.
• Review provider gap uploads and serve as point of contact for groups.
