Telligen, a national population health management organization, seeks a Quality Improvement Advisor to coach hospitals and other providers in improving patient outcomes and safety. You will lead improvement activities, analyze data, and partner with stakeholders to advance evidence-based practices.

Competenze

• Bachelor's degree in public administration, public policy, public health, or related field or 10 years healthcare experience
• Healthcare quality improvement experience required
• In-depth knowledge of quality improvement principles and methodologies used in healthcare
• Strong interpersonal and communication skills with C-suite, clinicians and admins
• Familiarity with CMS quality measures, survey and accreditation standards
• Ability to coach and facilitate improvement activities with providers and partners
• Active EUA ID requirement for CMS access and ICT account onboarding

• Collaborate with nursing homes, hospitals, and outpatient practices to identify needs and drive measurable improvements
• Partner with local, state, and national organizations to connect providers to initiatives and learning opportunities
• Analyze regional and provider-level data, facilitate learning collaboratives, and provide technical assistance
• Engage with partners across healthcare continuum while aligning with state and federal quality objectives
