Job summary

Comagine Health seeks an Improvement Advisor to support quality improvement and population health across Idaho, Washington, and Oregon. You will work with outpatient clinics, hospitals, and community organizations to expand access to evidence-based programs and strengthen care coordination.

You will lead quality improvement coaching, stakeholder engagement, and project implementation to build sustainable processes that improve access to care and health outcomes.

Qualifications

• Bachelor's degree in public health, health care, or a related field.
• Four or more years of experience in health care, including quality improvement work.
• Two or more years of project management experience.
• Knowledge of quality improvement methodologies and clinical workflows.
• Strong communication, facilitation, and relationship-building skills.
• Experience collaborating with health care professionals and community partners.
• Ability to manage multiple projects and meet deadlines.

Responsibilities

• Provide consultation, coaching, and technical assistance to health care providers and community partners.
• Design and implement quality improvement strategies for care coordination and population health.
• Evaluate clinical and operational workflows and identify improvement opportunities.
• Develop sustainable improvement plans and measure progress.
• Build organizational capacity through education and hands-on support.
• Coordinate with partners to implement community-clinical collaboration initiatives.
