Job summary

Randstad USA is seeking a compassionate Community Resource Coordinator II to support Health-Related Social Needs (HRSN) under a Medicaid waiver. This remote role bridges healthcare benefits and social determinants of health, coordinating case management and authorizations while engaging members and providers.

The position requires a High School Diploma or GED and at least 1 year in customer service or healthcare coordination.

Qualifications

High School Diploma or GED required.
1+ year of professional experience in customer service, member-facing roles, or healthcare coordination.
Excellent empathy and compassionate communication with vulnerable populations.
Strong time management and ability to balance intake work with live communication.
Remote work self-starter with ability to navigate multiple software tools.

Responsibilities

HRSN & SDoH Case Management: Coordinate and process social determinants of health (SDoH) service requests under the Medicaid waiver program to connect individuals with essential social support benefits.
Authorizations & Intake: Review incoming referrals, verify eligibility, and build service authorizations accurately within proprietary software systems.
Member & Vendor Engagement: Serve as a primary point of contact for members and community service providers/vendors to resolve inquiries, coordinate resources, and guide individuals through service delivery.
Multi-System Navigation: Manage high-volume request queues across several internal databases and software tools while maintaining data integrity.
Compliance & Turnaround Times: Consistently meet or exceed stringent state-mandated turnaround times (TAT) and quality assurance metrics in a high-volume environment.
