Job summary

Qualifications

• Associate nursing degree or equivalent clinical experience.
• Current, unrestricted license to practice nursing in the United States.
• Active Virginia RN License.
• 3+ years' experience in an Acute or Managed Care setting.
• Experience with EMR systems.
• Ability to meet deadlines with little to no supervision.
• Ability to work independently and meet productivity expectations daily.
• Demonstrated solid analytical and interpersonal skills.
• Demonstrated computer literacy.
• Knowledge of Chronic Illnesses and medications used to treat those diseases.

Responsibilities

• Provide telephonic nursing services, including transitions of care and medication reconciliation.
• Coordinate with schedulers, pharmacists, providers and social services as needed to support patient care.
• Educate patients to support self-management and behavior modification.
• Support quality improvement processes and case management.
