• HS diploma or the equivalent.
• Minimum of 6 months experience; or any combination of education and experience which would provide an equivalent background.

Responsabilidades

• Gathers and records appropriate member information via telephone.
• Encourages members to participate in Care Management programs by providing information about features and services.
• Records call details and uses call scripts as needed based on client requirements.
• Directs member inquiries to appropriate clinical staff.
• Initiates referrals and manages consults from clinicians for participants in Care Management.
• Gathers information from hospitals, health plans, physicians, patients, vendors and other sources.
• Contacts physician offices to obtain demographic information or related data.
• Mentors less experienced Health Program Advocates.
• Possesses ability to multi-task across calls, texts, and queues while taking notes and talking to customers.
• Maintains focus during long periods of sitting and in a fast-paced environment.
• Demonstrates empathy and problem-solving to resolve caller issues.
