Penquis is seeking a Full-Time Health Home Coordinator to provide care coordination and case management for adults with approved mental health diagnoses. This primarily remote role includes required travel and in-person meetings in the Dover-Foxcroft and Bangor area as needed.

Responsibilities include collaborating with a multidisciplinary team to address behavioral and physical health needs, conducting clinical assessments, and tracking required review and treatment dates for a year ahead.

Formación

• High School Diploma and MHRT-C certification required.
• 2-3 years’ experience working with children, adolescents or adults with disabilities.
• Valid Maine driver's license with reliable transportation and flexible hours.

Responsabilidades

• Coordinate care for adults with mental health diagnoses, integrating physical health needs.
• Collaborate with supervisors and clinical team to provide comprehensive services.
• Provide accurate clinical assessments and annual assessments.
• Track Mainecare review dates, assessments, and treatment plan deadlines for a year.
• Support crisis intervention and follow-up with crisis plan development.
