Morrison Center in Maine is seeking an Adult Care Coordinator to manage an active caseload and coordinate comprehensive services for adults with developmental disabilities within the Behavioral Health Home program.

You will conduct home and community visits, lead team meetings, and connect members with health, housing, employment, benefits, transportation and community resources. Strong organizational skills and the ability to thrive in a fast-paced, team-oriented setting are essential.

Formación

• Bachelor’s degree in a related field
• MHRT/C eligible
• At least 1 year paid experience with behavioral health, intellectual or developmental disabilities
• Strong organizational, writing, and time management skills
• Ability to thrive in a fast-paced environment with multitasking and problem-solving responsibilities
• Care coordination experience preferred but not required

Responsabilidades

• Maintain an active caseload and monthly contact with members.
• Conduct home, community, and office visits to support member engagement and service delivery.
• Lead interdisciplinary team meetings and coordinate communication among providers, supports, and community partners.
• Provide comprehensive care coordination services to adults in the Behavioral Health Home program.
• Connect members with healthcare, housing, employment, benefits, transportation, and community resources.
• Monitor progress and adjust care plans as needed.
• Complete comprehensive, person-centered assessments in line with DHHS and program requirements.
• Promote informed decision-making, self-advocacy and independence.
• Identify service gaps and coordinate with partners to address barriers to care.
• Maintain accurate documentation per DHHS, MaineCare and Morrison Center standards.
• Participate in quality improvement, audits and compliance.
• Serve as liaison among members, families, providers and community agencies.
• Travel within the community to support members and coordinate services.
